Name the scaffold or platform at the top. Where one sheet covers several, use the location column on each row.
Record every inspection
Including the ones that find nothing wrong. The report is the evidence that the inspection happened.
Copy it within 24 hours
To the person the inspection was carried out for, and note the date in the last column.

How often must a scaffold be inspected?
Before first use after installation or assembly, at least every seven days, and after anything that could affect its safety, such as severe weather. This applies to working platforms for construction work from which a person could fall two metres or more.
What must a scaffold inspection report contain?
The particulars in Schedule 7 of the Work at Height Regulations. Who it was for, the location, a description, and the date and time. Any risk found, the action taken, further action, and the name and position of the person making the report.
How long must scaffold inspection reports be kept?
At the site until the construction work is complete, then at an office for three months.
Who can inspect a scaffold?
A competent person. The regulations define inspection as the visual or more rigorous inspection by a competent person that is appropriate for safety purposes, including any testing.
Does a scaffold tag replace the report?
No. A tag shows the latest result at the scaffold, but for a platform covered by regulation 12(4) the written report with the Schedule 7 particulars is still required.
