A toolbox talk attendance sheet, sometimes called a sign-off sheet or signature sheet, records who was present at a safety briefing and that they understood it. This one is a single page: the site, date, time, topic and who gave it, twenty rows for names and signatures, the questions raised and the actions agreed. Editable PDF and Word, free to use and reuse.

No regulation requires a toolbox talk attendance sheet. The duty is to provide comprehensible and relevant information, and training when risks change. A signed, dated sheet is how an employer shows it did.

Compare the names with who is on site. Anyone who missed the talk is given it before they next do that work, and signs then.
Keep the sheet with the site records and add a row to the toolbox talk register so the planned and the given can be compared.
